Efficacia dell’ago a secco e dell’agopuntura nei pazienti con fibromialgia: una revisione sistematica e una meta-analisi Juan Antonio Valera-Calero et al. Int J Environ Res Public Health. 2022.
La fibromialgia (FM) è una sindrome che coinvolge dolore cronico, affaticamento, disturbi del sonno e compromissione della qualità della vita e del funzionamento quotidiano. Oltre alle terapie mediche e psicologiche, altre terapie tra cui l’agopuntura e il pungente a secco mirano a ridurre il dolore e la disabilità nei pazienti con FM. Lo scopo di questo studio era quello di studiare l’efficacia del agopuntura a secco e dell’agopuntura nei pazienti con FM per quanto riguarda il dolore, la funzione e la disabilità sia a breve che a lungo termine. I database MEDLINE, PubMed, SCOPUS e Web of Science sono stati sistematicamente cercati per studi randomizzati controllati che valutavano i dati di efficacia dei trattamenti di agopuntura a secco e/o per migliorare il dolore, l’affaticamento, i disturbi del sonno e la compromissione della qualità della vita e/o della funzione quotidiana. È stata eseguita un’analisi qualitativa che includeva la qualità metodologica e una sintesi sistematica dei dati. Un totale di 25 studi hanno affrontato i criteri di selezione. La maggior parte degli studi aveva una qualità metodologica accettabile. Quattro studi hanno valutato l’effetto dell’agopuntura a secco e ventuno studi hanno valutato l’effetto dell’agopuntura. In generale, entrambi gli interventi hanno migliorato il dolore, l’ansia, la depressione, la fatica, la rigidità, la qualità del sonno e la qualità della vita. Tuttavia, entrambe le tecniche non sono state confrontate in nessuno studio. Le terapie di agopuntura e agopuntura a secco sembrano essere efficaci nei pazienti con FM, poiché entrambe hanno ridotto le soglie di pressione del dolore, l’ansia, la depressione, l’affaticamento, i disturbi del sonno e la disabilità a breve termine. Hyperbaric treatment to treat infertility
It is estimated that between 15% and 25% of couples suffer from infertility. Among that percentage, 39% comes from the female part and 20%from the male part. The women’s fertility reduces naturally with age, this is because the quality of the oocytes and the ovarian reserve decreases. From the age of 45 years, the possibility of having a successful pregnancy is low. Indeed, a healthy and fertile woman of 30 years old has only a 20% of chances to get pregnant regardless of the method chosen. After the age of 40, the chances reduce to 5%.
Also, there are some cases of infertility, both feminine and masculine, that come from different pathologies such as endometriosis, in the case of women, and erectile dysfunction in male patients. However, the fertility field has paid special attention to hyperbaric medicine because of the benefits of oxygen.
Hyperbaric oxygen therapy (HBOT) produces a significant increase of oxygen in blood promoting a better functioning of the organism, stimulating angiogenesis, and regulating oxidative stress. The physiological effects generated by hyperoxia can promote a better scenario for fertilization to occur correctly and, enhance assisted fertility treatments.
In the feminine case, the HBOT contributes to improving ovulation and the quality of the ovule. Also, It is involved in the preparation of the endometrium and the quality of the endometrium for implantation. Angiogenesis and blood flow in the ovary is necessary for the process of ovogenesis, especially for the development of the follicle. Several trials have demonstrated that oxygen is necessary for the meiosis of the oocyte and those that enjoy a better blood flow have better quality to be extracted for in vitro fertilization. On another hand, infertility in men can be presented by genetic conditions, erectile dysfunction, or bad habits, such as smoking and obesity, affecting sperm quality. The mechanism of action of HBOT in sperm functionality is directly involved with the mechanism of action of oxygen on sperm mitochondria. These mitochondria, not only are necessary for ATP production, sperm functionality, and the movement of the tail for the scrolling, but also have an important role to keep the regulation of the oxidative balance, oxidative stress, and enzymes to regulate it.
In dysfunctional erectile, angiogenesis can reverse the disruption of vascular blood flow in the penis, which is usually present in 60% of such cases. It also reduces testicular ischemia-reperfusion damage so it could be used as a preventive treatment for secondary infertility in situations of testicular torsion and spinal cord injuries.
In conclusion, hyperbaric oxygen treatment can be a coadjuvant therapy that helps to prevent and promote infertility treatment by improving the fertilization period. Also, it generates a favorable environment for successful implantation.
Case report: Hyperbaric oxygen therapy at medium pressure to treat livedoid vasculopathy.
Msc. Alejandro Herrera together with Dr. Maria José Madriagal and Dr. Fabrizio Verdini, medical liaison of Biobarica Hyperbaric Solutions, developed a clinical case of a 26-year-old female patient with a livedoid vasculopathy. This skin disease manifests as recurrent leg ulcers, is characterized as a non-inflammatory thrombus occlusive condition and significantly impairs the quality of life of those who suffer from it. Treatment is usually complex because its pathogenesis is unknown, however, it was found that hyperbaric oxygenation treatment (HBOT) can work effectively as an adjuvant therapy due to the effects triggered by hyperoxia.
Research suggests that hyperbaric oxygen therapy at medium pressure is a rapid and effective alternative for the treatment of livedoid vasculopathy in patients who do not respond to conventional antiplatelet therapy, immunomodulatory therapy or oral corticosteroids.
The medical team postulates the case of a woman who, for the past 4 years, has been suffering from multiple recurrent episodes of ankle and foot ulcers. Since the patient reported recurrent dermatitis with small painful ulcers and 4 severe episodes lasting between 2 and 6 months, HBOT was prescribed as an adjuvant treatment.
She has been prescribed 60-minute sessions at a pressure of 1.45 atmospheres absolute in the Revitalair 430 hyperbaric chamber. The reason why it was decided to use the Biobarica chambers is that an effective pressure is achieved, but safe for the patient and it does not require high maintenance costs like the rigid chambers. During the first two weeks of treatment, the patient had 3 weekly sessions and in the following weeks, the frequency was reduced to twice a week. After 12 sessions, it was observed complete wound healing and a significant reduction in pain.
These improvements are mainly due to the hyperoxia generated by the hyperbaric chamber that counteracts cellular ischemia, the stimulation of collagen synthesis that favors the wound healing process and tissue repair, the analgesic effect and the stimulation of neovascularization. Healthcare professionals conclude that the systemic anti-inflammatory effect of HBOT, and the induction of fibrinolytic molecule synthesis in vessel cells, may have been involved in wound recovery.
Although more research is required to further investigate the benefits of hyperbaric oxygen therapy to treat livedoid vasculopathy, this clinical case has made a great contribution to the field of hyperbaric medicine due to the complexity of the pathology.
